Assuming she's been straining for a while, you need to get her to a vet immediately or her and the kittens will die. If the kittens aren't coming out, she needs a C-section.

Well you should be her 'coach' and tell her what a great job she's doing! It's her first time and she's probably a bit scared. . .I know I was scared having my first son. If she still has a tuff time at it look in the phonebook for a 24/7 vet emergency line or try telling a vet your problem and that you can't possibly bring her in to the office in her condition. give her time. Nature will take care of her. Keep a eye on her. Take a peek & see if the kittens are coming out head first. If she begins to have problems she will let you know. During labor she may get loud but if she has problems there will be no dout in her tone. If a kitten is backwards give it a real gental tug. once 2legs are poking out it will be fine. If she seems to have severe problems put her in a box & take her to the vet.

she'll be ok. generally cats dont' need help when it comes to birthing thier litter. but it is a slow process. the kittens might take along time to come out. if your really concerned then phone the vet. they should be able to talk you through it or recommend something.
