The phrase I know of is usually "putting two and two together to make five!"
This basically means that someone has taken a limited amount of facts, and built them up into a story which is totally wrong - for example, someone tells you that his new girlfriend is named Tracy, and then someone else tells you that HE has a new girlfriend, and she is also called Tracy. So, two Tracys. If you put those facts together and add a bit of imagination, you would believe that there was really only ONE Tracy, but she was dating two of your friends! But you don't KNOW that, you just put the clues together and come up with your own fantasised notion of the truth!
You are also putting 2 & 2 together and making 5 when you get a headache, and become convinced that you've got a massive tumour in your brain..!
