It is possible, you just have to feed so much premixed air forced down the fuel's throat that it just can't burn fast enough.
A bunsen burner can do that, which is what happens when you adjust the air inlet until it gives a bright blue.
If there's any yellow in it then its not quite 100%.
Almost any normal flame is grossly underoxygenated, the smoke, the soot, the smell, the orange-yellow color, wouldn't even exist if it oxidized completely, leaving nothing but CO2 and water vapor.

I just saw an article that explain this 100% combustion on MSN, it is possible with gas like natural gas, propane and methane, it was possible with alcohol(100%) no flame.
The people doing research are predicting that a water heater can be marketed in 1 to 2 years.

100% Combustion is not a reality. if it is so we can easily convert matter to energy. In every combustion, Some oxides and left overs are there. Heating or combustion is not a chemical process its a physical phase change or characteristic change.

Given enough oxygen it IS possible; the only thing left over is various oxides which is what combuston produces. Example -Smokeless powder used in ammunition
