d. I have never seen a waiter in a roadside hotel, however, I do leave a small tip (a couple of dollars or so) for the cleaning lady. That's what the envelope is for.

McDonald's- Nothing, you pick up and clean up your own food. Tips aren't charity.
A waiter- Any location- from 0-20%. Depends on service. Should not matter if it's road side or the best place in town. Good service is good service. Tips aren't charity. You do the good waiters a dis service by paying them the same tip as a waiter that doesn't care. 20% for the best and 0% for the person that needs a new profession. Some people just don't have the service personality.
Bell Boy- Shouldn't matter if it's 5 star or no star. Good service is good service. These guys can help you.
The best can tell you the good places to eat, places to stay away from, fun spots, how to save ... Tip them well- 10-20.
Average guys- just brings your bags in in one piece- 5
Poor service- not just I don't like em but someone that needs a different career- 0.00

A. None, its fast food! No tips required. If they offer great service, then whatever your heart desires.
B. Since you'd expect excellent service from a 5-star hotel, tip him atleast 15%. But that varies between lunch and dinner. (Usually you would tip more for dinner than lunch.) But of course, if you don't like the service, you don't have to tip as much.
C. Personally, I would tip both bell-boys the same amount. But depending on the situation, I would tip him more or less. I think 5 - 10 USD is more than enough.
D. I would tip 10 - 15%.
- Remember, waiters and bell-boys usually work for minimum wage. This means that their income is hugely based on tips! (In today's society, its extremely hard to live off minimum wage. Most people end up in debt due to insufficient funds.) It would be nice to tip them a little even if it wasn't the best service. But there are no certain amount that you HAVE to tip! It is purely up to you!
