Heh, I personally think that Daikatana is one of the worst of all time, though perhaps that's more because of the hype it generated on which it failed to deliver.
If you want to know more, check out Game Rankings - it's a pretty cool site, as far as I can tell, and has a list of both (source below). It's all based on people's opinions, but hey...so is this. :)

There are so very very many. Bible Adventures, Deadly Towers, and Wall Street Kid for the NES, Ultraman for the SNES, Mr. Mosquito for the PS2... the list of absolutely awful games is virtually endless.
Although he only covers one system, Sean "Seanbaby" Riley compiled a hilarious list of the 20 worst NES games. It's definitely worth a look.

Sorry, guys. The absolute worst game of all time is
"Aquaman: Battle for Atlantis". If I remember correctly, "Grabbed by the Ghoulies" is even ranked above this "game".
