What is a chelsea grin?

Is it the one done with a credit card, or the one with the razor, and what happens to people who have it done to them?

I've heard it called a Glasgow or a Chelsea smile. I have heard of the credit card thing but back in the day I think a blade was used to cut the mouth and then the person was 'hurt' enough so they'd cry out in pain, open their mouth and tear the sides.

As for what happens to them, I'd imagine reconstructive surgery is the answer!
