Call me crazy, but I think that anyone with a bit of common sense could have realized that putting a 230 pound man in a five pound harness was going to cause a bit of trouble.

He wasn't killed, At "Over the Edge"(a PPV back then), as the Blue Blazer, he flinched when he was repeling down from the raptures, which caused his safety harness to deactivate and he fell about 30 feet to his death
