I pay 35 a month for two tivos. I bought it directly from Tivo.com. Each box was $30 and they are mine. I pay a little more per month because I paid less for the tivo boxes itself.
It does not take long to set up at all, I got both mine up and running in half an hour. If you buy directly from tivo, the box that is shipped to you comes with everything you need and I mean everything. The RF splitters even. There is a large glossy step by step guide that is extremely easy to understand. I have the series 2 I think it is, the dual tuner box, so I can record two programs at once.
It is definitely worth it. I have had dvr from my local cable company and it just does not compare to tivo. Tivo is like a little brain for your tv, it is a genius invention and the most user friendly dvr system available. I would most definitely suggest buying a tivo, you will pay more but it is worth it.

I have 2 TiVos right now and they are the best invention to have ever graced the planet earth. I pay only $6.95 a month for both because the one that is in my room i got the lifetime membership, which they no longer offer, and the one in the living room is only $6.95 a month. We are thinking of getting the new TiVo HD which can store 300 hours of regular TV and 30 hours of HD. If you don;t already have TiVo you have to get it ASAP it will be the best purchase of your life!

I pay $0 and 0 cents because I purchased the life time service. This sadly is no longer offered, but my friend told me, TIVO called him and offered it to him, if he called a special 800 number. My friend has been known to be a complete lier, and usually without any reason, so I can not vouch for this fact.
That being said, if you CAN get a lifetime membership it is well worth it, just make sure you also purchase the extended warranty because lifetime membership is for the life of the machine (not the user).

I pay 12.95 monthly too. It took me a long time to set it up because I have a lot of peripherals attached to my TV.
I think DVR is worth it but not necessarily Tivo. If you have the choice, I would go with the DVR from your cable company (like Time Warner DVR).

mine is $12.95 automatically deducted monthly (and may i just add, it's the best $12.95 i ever spend :) )
i bought it at circuit city when there was a rebate on it, i think it ended up costing about $100. I just saw an ad, though, saying with store rebates you could get one basically for free. It takes about an hour to set up, and YES YES YES it is worth having. I can't stand watching "live" tv now. I've never met a person who wasn't in love with their TiVo. It is defintily worth the investment.
