I believe that providing that there was/is action involved, That the INTENT is what counts. We can never actually control the outcome of our actions, so the only thing that COULD count on the good/bad scoreboard (if there was one) would be intent.
(Intent without action is worthless)

obviously much depends on the context and what the intent and action where, but in the long term it is prudent to understand the action and effect as more meaningfull then the initial intent.

That's one great thing about Islam, it teaches that God looks at intention, so even if things didn't turn out so great, if your intent was good, it's equal to a good deed. I believe that.
Same for evil intent, even if it doesn't turn out, your intent is still evil.

It is for sure the intent....I try to think carefully before I do anything, but sometimes....I forget about the other person. Selfishness is something I do have issues with and am slowly trying to work through.
