You can Google for <"had had had"> (using the quotation marks) and find various torturous examples. Personally, I find them to be rather contrived. Just my $0.02.

He had an apple but he wished he had had a banana instead.

There is no way to use "had" in a sentence three times in a row and it make any sense at all.

Joe was writing a paper he had "had had enough already". It was not a complete sentence.
