Btw, it is taken from this passage:

Distance (46%) and fees (19.3%) are the major barriers in accessing health facilities. Lack of staff is a major issue especially in Alasa (Tugala Oyo). If distance is related to available means of transport is not clear as only the most important barrier could be chosen. However when comparing the given distance in km. with distance as barrier 58% perceive a distance below 6 km. as a barrier.

Dunno where you got this sentence from, but I can only make sense of it if I assume that there's a typing error and the writer means 'carrier' not 'barrier,' where 'carrier' is a method of transport (car, truck, plane, etc.).

In that case the sentence might be better written as:

'Whether or not distance is related to the available means of transport is not clear, because only the most important carrier could be chosen.'

Whoever wrote that should be taken out and shot. Hope I guessed right.

OK,

If distance is related (to have relation to or to have reference to) available (current) means of transport is not clear (understandable) as only the most important barrier (obstacle) could be chosen.

So really this means:

If distance has a relation to the current means of transport (cars, buses, etc.) it is not clear to understand the most important obstacle that could be chosen.

But yes, it is very hard to understand.

Distance could be made shorter by available transportation. Barriers can be chosen only when they are very important.
