Spend the day with him and share a drink or two (if you both imbibe), share old memories, and leave the bottle with him when you leave.
Happy 90th, grandpa!

Cologne
Wine or whiskey - if he drinks
A nice picture of your family in a nice frame
Shirts
Chocolates - my grandpa really enjoyed his chocolate
Favorite pipe tobacco - if he smokes
It's little things that mean the most, you'll know when you find what you want for him.

A basket filled with stationary, envelopes, stamps, and a nice pen. Lots and Lots of stamps. It is handy for paying bills and they always need them.I have never known an elderly person who didn't appreciate them.
