my statistics project!!!!!?

i have picked up a statistics project on smokin in different age groups.and its effect on health.can anyone tell me the the questions that i should include in the questionnaire,how to present the data and how to conclude the project.im doin a project for the first time and have no idea on how to do it.thnx in advance for all those who provide information.

Frequency, how many sticks per day, at what age did they start smoking, gender and present age, any noticeable changes after smoking. Now , you prepare the questions so you can apply the rules of preparing the questions as lectured on by your teacher. Be a good student and do most of the work. Learning is like that.
