There may not be a lot of poems that say "I am Canadian" in words, but consider these.
This one is not specifically Canadian, but it is a great "I am (any nationality) poem ... it's from the Lay of the Last Minstrel by Sir Walter Scott ....
Breathes there the man, with soul so dead,
Who never to himself hath said,
"This is my own, my native land!"
Whose heart hath ne'er within him burned,
As home his footsteps he hath turned,
From wandering on a foreign strand!
If such there breathe, go, mark him well;
For him no Minstrel raptures swell;
High though his titles, proud his name,
Boundless his wealth as wish can claim;
Despite those titles, power, and pelf,
The wretch, concentred all in self,
Living, shall forfeit fair renown,
And, doubly dying, shall go down
To the vile dust, from whence he sprung,
Unwept, unhonoured, and unsung.
Then there is "I Like Canadians" by Hemingway (one of the links).

There is the song "Something to Sing About: This Land of Ours" by Patrick Lashmar.
Perhaps the best known Canadian poem is "In Flanders Fields" by John McCrae.
The poems of Bliss Carmen, E. Pauline Johnson, and Sir Charles G. D. Roberts are certainly classic examples of Canadian poetry.
One of my favorite Canadian poems is by a Newfoundlander, E. J. Pratt ... Erosion
It took the sea a thousand years
A thousand years to trace
The granite features of this cliff
In crag and scarp and base.
It took the sea an hour one night
An hour of storm to place
The sculpture of these granite seams
Upon a woman's face.
I can get carried away with poetry. :)
EDIT: I had to include this one, Leetle Bateese by William Henry Drummond. It is one of our favorites - my husband and I both shed a tear every time we read it. It's at the last link.
EDIT 2: I found a poem, also by Drummond that certainly says I am Canadian in a beautiful way. It is De Nice Leetle Canadienne. Link included.

Hey, I'm not a lumberjack, or a fur trader....
I don't live in an igloo or eat blubber, or own a dogsled....
and I don't know Jimmy, Sally or Suzy from Canada,
although I'm certain they're really really nice.
I have a Prime Minister, not a president.
I speak English and French, not American.
And I pronounce it 'about', not 'a boot'.
I can proudly sew my country's flag on my backpack.

There was a Molson's Canadian beer commercial of that title that was one of the most sucessful commercials ever in Canada. I'm sure the beer company would be glad to email you a copy.
