...I kinda need it really soon...can i just get it from my doctor or school nurse? or do i need to do something else to get an official copy?

Go to the last hospital or clinic u been at and ask for a copy. Be sure to bring at least 2 photo I.Ds, two pieces of mail with ur name and home address on there, and your birth certificate just in case.

You should be able to get them from your doctor and if not your doctor then definitely your school because schools are required to have that information available...

I was able to get a copy from my old school and I hadn't been there for over 15 years... (i had to request them to go into the archive and it took 2 days but was EASY).

Just call the school and talk to someone in the office.

Your previous school may be able to fax you a copy. If the new school requires an OFFICIAL record, you could call the health dept. or dr. office where you received your vaccinations.

Your md should supply you one -if he gave you your vaccinations- just ask for a copy- you might have to sign a release of information form= D

Go to your pediatrician that you had when you were a child and they will have them on file. Also hopefully your current Dr. will have them if you transferred your record. Good Luck!
