Her brother threatens violence against her elderly father and mother. He threatens to hurt himself when he doesn't get his way. A few months ago he broke his fathers rib in a fight. He's not even supposed to be living there. He's threatened to "shoot myself in the heart" if they tell the police he's living there. It's causing serious problems. I feel the need to help. I just don't know how.

Call the police when hes not near you or expects it, then they can pick him up without any warning and possibly avoid any violence against the family. He needs help, serious mental help.

In my experience, there is not a whole lot that can be done.
If he does not want help then nothing ANYONE says or does will change anything.

The only thing that CAN be done in these cases is to protect the rest of the family, himself and perhaps others from him. That is to say, that he needs to be removed for the home in whatever way necessary and, as harsh as it sounds, if that means calling the police then, well, that is what has to be done. Since this is your GFs family and not your own, you perhaps can be a voice of reason for her, and depending on your relationship with the family, with them as well.

When you are embroiled in this sort of thing it is hard to see the big picture, and perhaps an open and honest dialog, beginning with your GF, to show how unless he is willing to accept help (and then follow through with it) that something much more drastic must be done.

My brother, at one point, was out of control. He was stealing from us and he was violent and finally my mother had to have him put in jail. For my brother, that was a sobering experience and his 120 days in jail were enough to convince him that he needed some help. Of course, many people are not as easily convinced as was my little bro but at least when he was in jail we knew we were safe and that has a HUGE effect on every part of a person's life.

These problems affect everyone negatively and no matter how much you love someone you cannot allow them to ruin everyone life, it is simply not fair to the rest of those people involved.

It is called "tough love" and sometimes it is the only way.

Good luck to you, your GF and her family, no matter how you slice it you are all in for a bumpy ride

He's acting more spoiled than an identifiable mental illness. He does need some help though. A stint in juvenile hall maybe. Not revenge, but some discipline.

Call the cops anonymously. In the US, "elder abuse" is a felony and it can be physical, mental or even financial.
