There probably isn't one single answer to this. Before bathing, cologne and such became popular, people just smelled like people and nobody knew any different. One tale of the Vikings was that after rowing and sailing hundreds of miles to raid a village in England they would row up a river and being covered in salt from the sea they would wash themselves and all their gear and clothes. The English girls they kidnapped, or just consorted with, noticed that they smelled better than the unwashed English men who were more apt to cover up their BO with various scents. As technology, plumbing and leisure time increased, bathing and use of cologne became more prevalent. One thing I've seen and done while backpacking is a smoke bath. You put some green pine boughs on the fire and let the smoke get all over you. This makes you smell like pine smoke instead of BO.

Soap, a lot of soap. Stink is much better removed than covered up.
