Some Wiccans believe in reincarnation. Some believe in the Summerland (something like heaven though everyone goes there when they die). Some believe we just die.
I believe that everything is made up of energy, including matter (which is taught to us by science). The last of our energy is given away in the form of heat. And so that energy becomes part of all other energy of the universe. Our bodies decay and feed other living things so that the life cycle can continue.
Therefore we never really leave, our energy only changes form. But we no longer have a consciousness.
Ghosts CAN exist for a while but they don't last forever which is why you'll never see a 5000 year old ghost. Eventually, their energy dissipates too.
So the answer to your question is that every Wiccan believes differently.

All of the above.
Different Wiccan traditions base their beliefs on different regional mythologies\religions.
Some Wiccans believe in the Summerlands, kind of like a heaven, based on European mythologies. Quite a few believe in reincarnation. Most will believe in ghosts\spirits or at least some form of live after death.
Personally I believe your afterlife is similar to your life - you are thrown in to it and muddle your way though in a variety of ways.

Wicca has no cut-and-dried set of beliefs, except that it urges its followers to non-violence.
