I hope you're joking. Feed your son 5 meals a day, very small meals, constituting of 60% carbohydrates 20% protein and 20% fat (keep it healthy fats like in fish and nuts). Keep each of the 5 servings of food around 500 Calories. The less time in between "meals", the higher your son's metabolism will stay, which means his body will burn calories even if he's just sitting doing nothing at all (like math homework). Encourage him to exercise even though being 200lbs is surely impeding that, it doesn't mean he can't go for walks and play sports. Make sure he's getting enough sleep, at least 8 hours, push for 10, force him to sleep if he won't, he's 10, he isn't in charge. The sleep will help both with his appetite and his focus on homework, or anything else. If that doesn't start to help him SLOWLY lose weight (less than a pound a week) over the course of the next two months, you need to take a class in nutrition or get him a proper doctor, because if your current doctor hasn't already told you how to keep his weight properly managed they aren't doing their job. The chart probably doesn't even go to 200lbs for a 10 year old, if I recall correctly mine stopped at 180. Good luck.
