It's not boiling a second time, it is reacting (breaking down, probably) to the leaves you threw in the water. If you have a thermometer, put it in the cup before you add the tea, check the temperature, and then add your ingredients. The temp. won't go up any higher than in the beginning.

I've heard a lot of people say that only distilled water will super heat, and only in a very very clean, nearly pristine container. That's a crock! I've had it happen several times with just plain tap water (which at times is downright crunchy with lime) and lord knows my dishes aren't pristine. I had a bowl of water that erupted, soaked the inside of the microwave and shot water back into it to the point I thought the microwave was ruined. I came away with minor burns and much wiser.
Just always ALWAYS beware of water in the microwave. Assume it could be super-heated and be very careful handling it until you've put something else in there with it.

The water has been super-heated. This can be due to being heated in a smooth sided container. You can put a wooden stir stick in the container, give it a vigorous stir and that can help prevent the boiling up effect.

To me apparently like you're putting forward...you pre make the bottle and then set the bottle in a bowl of boiled water to warmth it up. in my opinion...I purely placed extremely warm water from the tap in a bowl and position the bottle in it to warmth it. The water is already warm sufficient, no favor to warmth it.

Because it is super heated(over heated past the boiling point). Whatever you do don't put a metal spoon in it or it could explode and burn you really bad.
