Its a bruise. The sucking breaks many small blood vessels which are near the skin surface and they bleed into the tissues giving the hickey the typical reddish mauve appearance....;

It's a bruise caused by sucking on a body part, usually the neck or collar bone. You can get hickies anywhere though, with enough effort. I've had a hickie on my lip, and I have friends who have had them on their chests and ears. For most people, it takes a little time and a LOT of sucking, but if you bruise easily, watch out!
