Chronic sinusitis develops for a reason - to free you of the infection underlying problems must be corrected. For example - do you have swelling in the nose, a polyp, or a cyst which blocks the sinus opening? Nasal swelling is common and it is also a cause of eustachion tube dysfunction. It is easily treated with a corticosteroid nasal spray. The choice of an antibiotic is easy as only a fluoroquinolone (due to their bone penetration) will do - such as ciprofloxacin, levofloxacin, and moxifloxacin.

i can sympathise with you. i have had chronic sinusitis for years.
i have tried every treatment, but to nothing reallly cures it for good.
i find the only medicine that works for me is sinutab, and steam inhalation. but recently, i used otravine nasal drops, which were very effective, and i am also going to try what they call the HOPI CANDLE treatment, which is meant to help sinuses.

If you are really blocked up you may need a course of oral steroids (eg prednisone) to clear it up. After that an allergy medication and nasal steroid should keep you clear
