WI saint is right. However, the order is not. For example, his #5 should be #3, #1, etc.

The radiator itself is the last thing to consider.

1. clogged radiator
2. cooling fan inoperational and/or coolant temp sensor failure
3. Low coolant
4. weak radiator cap (check lbs. and go up if available. I.E. 13
lbs to 16 lbs.)
5. sticking thermostat

Start with the electrical, and go from there, as the fan and/or temp sensor is most likely at fault.
If you have never replaced your radiator cap, do it. It is CHEAP insurance. Higher lbs. mean that the cap can withstand higher pressure/higher heat before automatic pressure release.

Cooling fan is not coming on could be the sensor for the fan or the fan motor maybe frozen . Also check your fuses sometimes it blows

your radiator fan could damaged, it might not be turning on. also your radiator could be low on fluid or need to be flushed.
