during extraction of lower d to four years child her d has sharp abnormalities at its distal root?

the gingiva and the alveolar mucosa are damaged (9mm cut toward the distal) i described to her antibiotic 250mg three times and analgesics and dismissed her and i will see her tommoro what can i do for her in the next visit ,is it necessaryto make stitch or not ?
