Following , as normal causes , could be thought about :
=wrong technique (preparation of the area etc)
=wrong person (lack of expertise)
=wrong area (wrong choice of area)
=wrong equipment (non-sterile or improperly
sterilised syringes and needles)
wrong technique = technique involves the cleaning of the area with betadine, and alcohol with sterile cotton swabs etc. if not properly done, chances for injection abscess is there .
wrong person = there is a lot of difference between injection done by a doctor , staff nurse and a nursing asistant ..agreed ?
wrong area = all medicines cannot be injected at the deltoid area. muscle thickness, how much irritant the medicine is .. etc makes difference ...eg: Tetanus Toxoid causes vague discomfort for couple of hours after the injection. such medicines when given at a more bulky muscular area, it is better !
wrong equipment = the words explain by itself ..
If at all it occurs, early diagnosis is very important ! For this "early diagnosis" to be made, proper "past medical history" of the patient is very important !

Injection abscess could be due to improper sterilisation of the injection area, wrong site, wrong route of administration. Eg if a medicine is supposed to be given intra-venously and if it is given intra muscularly this might lead to injection abscess because of the irritant nature of certain medicines.
You dont get an abscess immediately. First you may have a hard indurated swelling. The swelling then gets infected. Abscess always means infection.
The abscess may sometimes require incision and drainage of the pus.
