I am going through this procedure right now. I had to provide my birth certificate, drivers license (or other photo, signed ID) and they wanted my marriage certificate (even though I have been divorced for many years - they need this if you are a woman, in order to show the legal change of your name if you took your husband's name). I had my divorce documents, but they wouldn't accept them and said they needed to see my marriage certificate, even though I had given up my marriage certificate to the divorce lawyer and never received it back. So, I had to go apply for a copy of my marriage certificate and pay another $27. As well, my passport photos were not "exactly" correct, so I had to go back for re-dos of my photos. A lot of bullsheet, actually. Now, I have the marriage certificate and the re-done photos and will return to try to get my passport. Wish me luck!

Pretty simple. When I applied all I needed was my original birth certificate (no copies are accepted).

Just take you BC to your post office and they will take your picture (Polaroid type) and pay them the current price for the service. It was less that $50 two years ago. If you want it sooner than the 6 weeks it normally takes I heard the cost increased from $60 to $80.

I received my passport and my BC in just under 2 weeks.

to get the paperwork for a passport you go to your nearest post office. they should have the forms or give you the forms to fill out to get your forms. it costs around $100 to get everything started. you will need 2 forms of a government issued i.d., such as a drivers license, social security card, marriage license. you might even need your original birth certificate. the people at the post office can help you figure out what you need to do.

you need to go to the nearest post-office who carry the form or download it on the Internet

if not an American citizen ? you need to get the form in an Embassy of your country nearest you .
