Say What are you up to? How was your day? How was your week? How was your weekend? What's going on where you're at? How's work? How's school? How is your significant other? How's your family? Have you seen any good movies lately? Did you watch that episode of (name a TV show) last night? What do you think of (song or band)?
You could get even more creative. Where would you go if you could take a vacation anywhere in the world? What would you do there? What would your ideal job be? If you could get any kind of education, what would you choose?
Ask questions to which there are answers (not toughies) that will lead into a conversation.

it really depends on how well you know the person.
for me, it could be the name of the person, a simple conversation-starting poke, a joking "how are the kids?" (i'm in high school, my friends don't have kids.) maybe bring up an important event.
if it's someone you don't know very well, a "hey, how are you?" kind of thing works. it really depends on how outgoing you are and the level of which you know the person.
