Sounds an awful lot like an allergic reaction to something eaten or introduced into the immediate environment. If Benadryl or a similar antihistimine relieves the problem, I think he's got some kind of allergy to trace down with a specialist.
If it happens at roughly the same 3 times each year, perhaps an investigation into what allergens he might be exposed to (something in bloom?) is in order.

likely an allergic reaction. You'll need to analyze everything he's been in contact with prior to the occurence, probably within an hour or so of it happening. It wouldn't have to be something he ate. Bee stings will cause the tongue to swell and the throat to constrict.

Is it something he is eating or could it be blooming? Yes, It could be anything from grass, trees, molds.
May want to be tested for allergies.

Mine does it if I use it to hard on the wife. Also she use to suck on my tounge,then I told her to be easy. Hey its just love making.
