No american cities will ever be nuked by any external enemy.
If 50 or so years of Cold War and several close escapes with 3 very close scenarios at its apex then forget it now. The MAD(mutually assured destruction) premise will see that off, as it has so far. Forget what you see in the media, how diplomats and generals and presidents etc talk tough. Even the diehard psychos on the Russian and US sides never did it.
Say Iran gets atomic then thermonuclear weapons. All of a sudden they have a great power and influence in the world as they cannot be threatened with invasion. If they hit Israel, they all die too when the Americans smash them with conventional warfare and when the winds send the toxic dust back to to Iran and crops fail etc. Remeber the nuclear accident in the Ukraine, several Euro countries were affected so no nation would ever consider doing it and the costs will always outweigh the benefits. The UN community is useless sometimes, but one thing it will not let happen is nuclear war, and if any nation did they would be cut off in isolationist fashion from the rest of the world.
In playing along to answer your question:
All the big facilities would have to go first, and to start by bombing population centres would be a poor decision.
Structures of command, ie, Washington DC(Pentagon) and major bases. The B2 Stealth base(real good idea to park em all in one spot, lol), The massive runway runway(satellite photos) at Area 51/S4(cos who knows what the next US advanced weapon is!?) all the main space facilities and New York City for the financial structure,capacity to wage extended war. I would definitely destroy Hoover Dam and all the same sort of targets, ie, power generating facilities, also the main manufacturing facilities...and would not waste nukes on population centres...
The US Navy is way ahead of second most powerful Navy and it's 15 carriers as well as fleet arms would be one of the first targets also(in port hopefully), as well as many ICBM capable subs..
and I totally disagree with the population centres as target unless it's NYC and DC for the reasons stated.

Depends on who's attacking and their capabilities. If it is North Korea, with limited nukes and range on it's ICBMs, then expect Seattle, Portland, San Francisco, Los Angeles, and Honalulu to be targeted.
If it's a country with many nukes, such as Russia, they'll probably just blanket the entire US with nukes, as well as NATO nations.

None. if there was a nuclear missile attack, ours move fast enough in response that whoever fired it would be dead before their missile got here. If it was a terrorist attack, in truth, no one pace is really more likely that another, as there are many diff groups of terrorists out there, and they all have different agendas and ideas as to what a good target would be.
