You do not need to be turned down by anyone or call anyone. Just fill in the blanks and answer the questions and you'll be able to print out the report.

If you have been turned down, you have the right to request a current report free of charge. But you need a written report in hand, I believe.
If you apply somewhere, it does take points off your current rating total, but at least if you were turned down, you would have the right to get a copy
