I think the best way to ask them is to just come out and ask them. The person that you are asking should be some one that you are comfortable taking to any way and they shouldn't have a problem writing one as long as you ask nicely.

You should ask very kindly, and make an effort to do so in person. Also, if you have any relevant info for the writer (for example, if it's a letter for a scholarship or something), you should give her that, too. It would be polite to inform the writer of the deadline, as well.

"Have you appreciated the work/service/job I did for you? If so, could I trouble you for a letter of recommendation?" for a service you provided.

For a school or job, "I have always respected your opinion greatly. I am thinking of applying for [XYZ]. Would you feel comfortable putting something in writing recommending me for [XYZ]?"

You should have something already written up or available in the event they say, "I'd love to, but I just don't have time."
"Knowing your busy schedule, I took the liberty of preparing something for you - would this help?"

Well, just go up 2 them & ask them, politly, if they may write a letter of recommendation for you.

Be polite, and let them know why you're asking.

Usually I tell whomever I'm asking that I value their opinion and feel they know my skills and strengths, and their input would be an excellent compliment to the application.

Just ask them and be sure to tell them why and what it's for. Most people would actually be flattered I think.
