What would a doctor see in a sonogram that would warrant a biopsy?

They have ruled out fibroids and the uterus is enlarged and has a rough surface.

Unfortunately, it could be almost anything. The interior of the uterus is a difficult surface to image, due to it being soft and irregular, so surgical biopsy isn't so unusual.

The only thing I can think of is endometriosis.

It depends on what they did the ultrasound on.
