Put your hand on their arm, lean over, look them in the eye, and quietly but firmly tell them "I think you just offended somebody. Let's change the topic."
Dirty looks they can ignore, but a flat-out (yet polite) statement that they are rude is something they can't ignore.
These people are usually either spiteful and mean, and won't care that they are rude (in which case you've at least taken a stand against them), or they incredibly obtuse and don't understand why their comments are rude (in which case they will bluster and try to prove how they didn't mean to be offensive, and you can explain to them where the line between rude and polite is).

You'll have to be brutally blunt with these types. The problem is, they're so into themselves, they don't see what is really going on. Subtle hints such as dirty looks will merely fly over their heads. To get your message through, first wait for him/her to act up. Then, tell it like it really is- straight to their faces. Try telling him/her off in front of others if possible, for added effect. And give specific examples as to what exactly you found offensive. I knew someone like this years ago and had no choice but to do what I did above. He might still be pulling his stunts as we speak, but at least he won't try his garbage with me anymore.

oh the type of humor where it's really not funny and at someone else's expense? yeah...can't stand ignorant people like that...
Just call them to the side and tell them that their behavior, trying to be funny at someone else's expense is not funny. it's rude.
It;s funny only if two people are laughing...and it';s not a personal joke. or making fun of other people.

Make sure you don't laugh at their attempts. (Sometimes it's a reflex to give a courtesy laugh.)
I would say something like, "I can't believe you just said/did that." It's a good all-around way of making people understand they are inappropriate.

be bluntly honest with these people - that's the only way to get through to them - tell them they aren't funny and that they're rude and hurt pepple's feelings and that it annoys you

Ignore them, if they keep harrassing you, say the normal pleasantries and then tell them you're off to do something else, or busy.
