I think this is a pretty obvious answer.
First, most Europeans and Americans feel threatened by Islamic countries. Also, the Pakistani government has made statements and taken actions which seem threatening (nuclear tests and the conflict with India, rumors that Bin Laden is in Pakistan, etc).
I am sure that even more secular Islamic countries, like Turkey, have seen a drop off.

I cannot speak for tourists from all countries of course, but being that I'm from New York, and an American, here are 3 basics:
(1) I do not speak the language.
(2) I cannot make use of a translation book, because the letters are very different than the characters I use, it would be nearly impossible.
(3) I would fear abduction for politic reasons, or for ransom.
If I were with a Pakistani friend, I might go along for a visit, it's hard to say.
