You need to do that with the camera, not with photoshop. The way to do it is with "Stroboscopic Photography." Unfortunately, you have to have access to a strobe light, one that flickers very fast. What you do, is you set your camera for a longer exposure, around 3 secs or more, and in a very dark room with a dark background (or outside at night) turn the strobe on. Then, as the skater, dancer, etc does whatever he does in front of the camera, he will only be visible in certain positions, thus making him able to be visible in several positions in a single frame.

Take a look at the photo, notice that the skater's hands are in different positions in each take... That effect is most commonly completed using multiple frame photography the camera takes a new shot every .02 seconds (or whatever it is set up for) the picture is snapped each time but the film does not advance, thus making all the action on one image. That is not an easy PhotoShop method to pull off from one picture.
