Depends how quickly they lose the weight, and if they are building muscle while they are losing the weight (the increased muscle tone helps with their body's overall appearance).
It also depends on how much weight they lose, and their skin type. Some skin types are more resilient than others, and shrink back right away. Some never do and you are right, surgery may be the only answer for some.

Depends on how much weight loss you are talking about--and how much skin is loose. To a point, the skin will eventually go back to normal, but sometimes people require surgery to repair skin problems after dramatic weight loss.
