The girl!
The girl will touch you first... it maybe on the arm or hand or your leg but she will touch you ever so briefly.
That is her signal to you that she would welcome your touch.
Thats when it is OK for you to hold her hand... And, it wouldn't hurt you to ask her permission first.

usuall the boyfriend.
that is not always the case though. for instance my current girlfriend did it first and i was okay with it
