This is a nightmare...you have to bee kidding, right?
Firstly????? There is no secondly, so there is no "firstly." And that is not a word that would be used in a questionnaire. November 14, 2006, as requested.
The next sentence is a total mess....The questionnaire ""Only come under my notice???? you have to be kidding.

Here is how it probably should read.
I apologize for the late completion of the questionnaire. I was abroad and returned on November 13, 2006.
Enclosed is the completed form. Please contact me if you need clarification.

you spelled questionnaire incorrectly the second and third time.
