ella huele mal or ella no huele bien are nice ways to say it
ella apesta or ella hiede (hee-eh-deh) (hiede means reeks) are rude ways to say it ,ella tiene mal olor means she has a bad odor
just as in English the context in which it is said determines if the verb is being used in one way or the other since it is said the same to say that she can smell something or that she is the smelly one, but mainly when you plainly say ella huele it means you are referring to her body odor
