will a hernia always bulge out during the "cough test"? also, has anyone ever had both a hernia surgery and gall bladder removed? which is a harder recovery time?

I had both removed (in three months time) three years ago.

The hernia was a little longer (by about a week) to heal. You have to get used to the way your stomach wants to hang and it pulls on the area just sewn.

Take it easy and don't do any lifting for a while.

Hernias do not always bulge out during a cough test, because that is only a test for a certain type of hernia. If you believe you have a hernia, have it checked out by a general surgeon. The answer to the recovery question depends on what type of surgery for both, and the severity of the problem. If you have a laproscopic surgery for either one, it is minimally evasive and you will be up moving around later that day. If you have a very acute problem with your gallbladder, the recovery time may be longer. If the Doctor decideds to have an open surgery, recovery for the cholecystectomy is a lot longer than a herniorraphy.

No they do not. Gall Bladder surgery is FAR WORSE.
