Yes, for Air travel starting in January 2007, for land and sea travel January 2008.

If travelling now then no, however birth certificate & D.L. are recommended.

Yes you do need a passport to get into & back out of Mexico.

I recommend bring your passport.. it's just a safe thing to have if you ever leave USA, especially going south, whether it be Mexico, or Bahamas or anything like that.

Yes! You can get them at the Clerk of Courts in your town. It takes 6-8 weeks unless you pay extra to get in 2-3 weeks
