the account is in your name, your parent is what some might call a "cosigner" if anything goes bad they talk to you, if you can't pay they go to parent and ask them to pay it for you, since you are under 18, leagally you can not have a bank account without a cosigner, just like you wouldn't be able to buy a car on payments without a cosigner

If your bank is like most banks, it will be your name on the card. Maybe they'll have Mom as a guarantor, someone to go to if something goes wrong, but for all intents and purposes its your card.

It can be in your name alone. Just depends on if that is OK with your parents. Their name will be on the account so will they let you have the card all by your lonesome?

It will be whatever name was provided at the time, or "Joe Smith or Jane Smith" (both). I believe you can request to have 2 cards, one with each name.

The card will be in yoru name, the credit agreement will be in their name. If you mess up your folks will get the knock on the door.

depends what they put. they could register it in ur name but still activate it under theirs. if they do that, ur name appears on the bill
