It doesn't have to be. My aunt is Amish and she doesn't refrigerate anything. If it is refrigerated in the the store or if you ever have put it in the refrigerator it then yes it must continue to be refrigerated.

Yes, mayo is made from salted egg yolk and while the salt is strong enough to keep most organisms in check anything with eggs should never be left out of the fridge once opened

Yes, if it's open..if it's not opened then you don't have to refrigerate it until it's open - you should read the jar it tells you right on the jar after it's open put in fridge..not trying to be mean just Staten the facts...

Once opened, yes it should be, but if you are like me, and sometimes make a late night sandwich after a few drinks, and then forget to put it back (thanks to the drinks) I wouldn't be too concerned. As a way of keeping mayo generally, yah put it in the fridge, but if you left it out a bit too long and are worried, I wouldn't be.

Yes...if store bought, you must refrigerate after opening. If homemade, you must refrigerate when you are done making/using it.

Yes after it's been opened. That's why they always say not to eat potato salad that's been sitting out awhile, it's the mayo that goes bad and causes food poisoning.
