Personal:
Find out what she likes for herself so it is personal to her. Otherwise, many church women give everything they have to the ministry and forget their own needs.
Examples: Flowers and the newest book from her favorite author, or if you don't know, a gift card to her favorite inspirational bookstore or other place to shop, that may have nothing to do with church, or a nice dinner at her favorite restaurant for her and her husband.
Formal, on behalf of the church:
A statue or bench with her favorite Scripture in the garden/park area; a permanent flower arrangement and vase, or large gilded Bible, to display at the entrance honoring her or her and her husband's contribution to the church; or a plaque to go with a contribution in her name to a cause very dear to her heart.
Above all, write a personal card thanking her for the little things that have meant most to you. Those letters and cards are often cherished as much, if not more than the gifts they came with.

It is nice to let people know we appreciate them. I'm sure this woman would love anything you choose to give her. Maybe make a donation to her favorite charity in her name? We recently took a collection at my Church for our music director and had a song written for him. He just loved it. Simple, sweet and well intentioned is all we need. Is there anything that you can personally make for her? Is there any job she does at the Church that you could assist her with? These little things always mean so much. I hope this helps a little.

This is a great question, I am friends with my Pastors wife and she recieved a beautiful afghan, she loved it, if you can't make them you can get one off ebay, cheap and new, also a lovely sweater would be nice or Bible cover.

How about a nice pillar candle with one of those glass plates to put it on? :) Not too "religious" but still shows appreciation.

You can make bath salts yourself out of epsom and essential oils and tie it up with rafia and make a nice basket out of it to remind her to take some time for herself
