I don't think she is trying to offend you, you could even take it as a compliment because she feels so at home with you and her sibling. I don't know what kind of person she is, so I don't have the right words, try talking to your significant other about how to handle it since she is his/her family. If it really bothers you, I would definitely resolve it quickly before you get too heated or something happens.

Yes, she is a bit too assuming. Just because you tend her children does not give her the right to walk in unannounced. I think you should ask her to knock or ring the bell. Explain to her that we need to teach the children manners or that it startles you and you would prefer that she knock or ring the bell prior to opening the door.
Our family typically, knocks or rings the bell, if the door is unlocked we will open the door and call out "Hi, it's me, can I come in?"
Or, if that does not work always keep you door locked, forcing her to knock. Tell here it is for the safety of everyone in the house that the door stays locked.
I assume you love your neices/nephews so you will have to approach this with care.

Keep the door locked, for quite a while then she will be forced to assume that it will be locked all the time and have to knock.

yeah sorta rude but you are taking care of her child so i guess she feels like she can just come on in
tell her politely hey knock before you come in so atleast i know your you and not some kind of creepy stranger
and if her kid isn't there KEEP THE DOOR LOCKED i don't know what to tell you about that

I don't think its rude,
if you were my sister in-law i would treat you like family and just walk in.
second thing is - your watching my kid, why should i knock, why would you lock the door,
do you have something to hide.
thats what I think.
only strangers knock and wait for an invitation.
family, is always welcome. and never have to ask permission first.
And besides , why would you wan to get up and open the door.
Or maybe your talking about people and you don't want her to hear you ?

Yes it is very rude. Maybe next time you can make a joke about it. Say " Don't you know how to knock"? Then maybe say i'm joking. Maybe it will work. Who knows.
