mix a carnation instant breakfast into the milk. This was suggested for my niece because the extra vitamins were good for her.
My son does have an intolerance to lactose. He used to throw up about 20 times a day before we realized what was causing it. If he had an allergy or intolerance, you'd know

If he has an allergy to milk it'll upset his belly when he drinks it, not before he's even tasted it. I think he's just being stubborn about the change. Talk to his doc and see what he suggests.
I think I'd try not giving him anything else until he drinks the milk. He doesn't have to drink a gallon of it, but an ounce or two would work for me, then he could have a little juice.
If throwing continues, I'd break that habit now. Make him pick it up and put it on the table or in the sink if he can reach it, or hand it back to you. He may start getting the impression that he can throw anything he doesn't like and get away with it. It could land you in hot water with family and friends should he hit some one with it, or something bigger when he gets bigger.

I wouldn't be too concerned. Eliminate the whole milk issue for a short time and try juice and water. After a bit, he'll give the milk a try again. I had the same problem with two of mine.

You should never give juice in a bottle....
But yeah- he's probably being stubborn. Try having mostly formula with a small amount of milk and build it up everyday.

Did he see you mixing the two? Maybe that's what it is. Try mixing it when he's in the other room. Give him formula with a small amount of milk and each day add just a little more milk and so on. Eventually it will be all milk and he won't even realize it.

None of my children liked milk when they were little. I think it is a power struggle. Keep giving it to him. Is it a bottle or a cup? That could be the real problem and not what is in it.

Well, juice is okay but high in sugar. As long as he's getting dairy in other forms, such as cheese, cottage cheese, yogurt, etc. he should be okay from a calcium standpoint.
Have you tried the stage 2 formulas? They might work better.

Give him chocolate milk. I don't think I've ever met a kid who turned down chocolate milk. After he gets used to it, you can try adding less and less chocolate to it, in case your not totally crazy for the idea. But it should work :D
