listen dear i will tell what i am doing with my 2 sisters, i start asking them b4 there birthdays about what they really need to buy and they will be happy to have it, its like making a Survey about what your sister need and belive me she will be more than happy to have something that she really want and wished to have more than having something she will never use, i know that you want to make a Surprise for her but better to get her something she love and dont let her feel that u asking her about what she wish for coz you want to buy her a gift, start talking with her about yourself and what u wish to buy or recive as a gift at your next birthday and then she will tell you what she need during exchanging thoughts together.wish to your sister a happy birthday and wish to you to buy something nice to her.

Buy something which you are sure that she would love it - something that she would be really happy when she opens the present.

If you have no idea, try to ask her discreetly. Even if she catches your hint and knows why you ask, she would at least realise how important her birthday is to her, and whatever you buy eventually, she would still be happy.

4 a 5/6 yr old: My little pony
4 an 8 yr old: a Barbie doll
4 a 10yr old: Game Boy or a book
4 a 13yr/ teen: Shoes or clothes or an iPod ;)
4 an adult: a book or a t-shirt
4 an old person: a silicone 'bra'

depending how old she is.
young - maybe a doll or some toys she's being eyeing for.
schooling - some school items (bag) or favourite books or book vouchers
middle age - vouchers of a departmental store
married - jewelley (silver) / same as middle age, a voucher

Well depends on her age, to bad you didn't put that in your question. But a good one would be a charm bracelet that way she can keep adding to it and remember that it came from you when she wears it.
