Well here's a new one for you. You could have a pinched nerve in the cervical spine, that's between the shoulder blades and the skull. It consists of six vertebrae and any one could have a pinched nerve from a bulging disc or a slipped disc. There could be some under laying factor also, like the spurs on the outside of the vertebre rubbing or locking on each other.
These nerves serve the arms and hands, some go to certain fingers, some others, they all go into the hands in the end, some go to the swallowing of your throat reflex.
Damaged, these nerves can cause shaking of the arms, or hands and pain, burning, irritation and soreness in the hands, the fingers.
They can cause the one nerve to run up the outside of the neck to just above the top of the ear and to the eye, sending pain into the back of the eye, creating a sort of dizziness.

I get this if I do anything too strenuous after a big meal. The only cure for me is to eat chocolate bars as quickly as possible. As soon as I swallow the first bite, I can feel the attack fading. So I'd go with the lack of sugar theory.
