Almost any unresolved mental trauma will reduce the brain's ability to function. Most people can remember only recent traumas, but these are often related to stuff that can be years old & lost to conscious memory, but the brain is still trying to deal with it.
Think of it like your computer running a whole load of unnecessary software. It slows down performance. The difference with the brain is that anything similar to the unresolved issue goes straight into the "I can't handle it" area.
Unresolved stuff still goes round in the memory & takes up capacity as the brain tries to come to terms with it. Once you're able to face meories of the trauma without it causing fear, then it is resolved & will release memory capacity & function again.
Counselling clients have completely changed as a result of resolving past traumas, childhood stuff often gets stuck there. I've seen a guy who had a 40 year old incident resolved & suddenly he could do normal stuff he'd only dreamed of.

the only way (seriously!) that i can think would interrupt your intelligence level in the way you suggest is being dropped on your head as a baby.
the reason why is that if you grow up and are intelligent & everything, and then something happens, you'll still be just as intelligent technically, you just won't be able to access the information in the way a luckier person might... take any mental illness you like as an example.
the head trauma that stops you from developing your intelligence are the ones that best fit your description.

Only brain damage in children can lead to learning disability. Meningitis can cause brain damage and loss of ability of the brain to function as it used to. The person will remain intelligent themselves but the brain just can't perform as they want it to. This is the case with alot of brain injury and head trauma, like motor accidents or falling from a horse etc.

no head trauma causes loss of intelligence. What head trauma can cause is the ability to access that intelligence.

its a bit complicated, brain acts as a whole and many parts serve each other ina way or another. but to be broad, the frontal lobe deals with life experience and defines character and personality. conclusive thinking (which in my opnion is the cause of intelligence) seems to freeze once the part called corpus callosum is cut. but actually its hard to tell.

Lobotomy. A piano falling on top of you. Anything that would kill just a whole bunch of brain cells.
