On domestic flights within the USA, no they are not required to have ID until they are the age of 18. If they have a student ID/ASB card, they can show that but it's still not required.
If you are flying internationally, then you need to check on the passport/visa requirements with your airline.

Based on previous experience, no. I believe if you're 18 and over, you'll need an ID. If you under 18, no ID is needed.
UPDATE: The TSA says:
We encourage each adult traveler to keep his/her airline boarding pass and government-issued photo ID available until exiting the security checkpoint. The absence of proper identification will result in additional screening.

I depends on where you are traveling to any overseas travel even some cruises are requiring a passport for all pass angers. Domestic travel is a little more lenient but regulations are changing. Check with the airline or your travel agent for specific requirements for all your travel restrictions and allowances. Good Luck!

I believe it is 18 or over. If you are 16, it is best to have it and show it when needed but anything younger than that, no you don't.

No. My son flew alone when he was 7 or 8 and we did not have any form of state id for him.

Yes, at least in Kansas they did. My brother and sister were roughly that age when they flew. All they needed were state issued ID cards. I think they just got theirs at the drivers license bureau.
